What is the medication sold under the name Rifadin? When is it used?

The brand Rifadin is a property of the French pharmaceutical concern Sanofi Aventis. The active ingredient of the medication sold under this brand is Rifampin, an antibiotic with pronounced anti-tuberculosis effect. The formulation was developed back in 1965 and is still widely used for the treatment of various forms of tuberculosis, i.e. lung tuberculosis or with the foci in other body parts, leprosy, gram-positive (especially staphylococcus) and gram-negative (meningococci, gonococci) pathogens. As since the creation of the medication more than 50 years had passed, for a long time already the formula can be used by basically all manufacturers of medications. Rifadin contraindications

Rifadin dosing

- Kids aged from six to twelve are given 10-20 mg per kg of the body weight. The highest possible daily dosage is 600 mg.
The duration of the course is individual and is chosen on the basis of the therapeutic effect and can be from one year and more. To avoid the resistance of bacteria, the formulation is used along with other anti-tuberculosis medications of I and II rows in the regular dosages.

Infectious and inflammatory diseases of non-tuberculosis nature caused by drug-susceptible pathogens - brucellosis, legionellosis, severe forms of staphylococcal infection (along with other appropriate antibiotics to prevent the emergence of resistant strains):
- Adults are appointed 900-1200 mg a day split into two-three intakes. The highest daily dosage is 1200 mg. After the disappearance of symptoms, the formulation must be used for two-three more days.
- Leprosy is treated with the use of the formulation and immune-stimulating medications. The medication is appointed in the dosage of 600 mg a day split into one-two intakes during three to six months. If necessary, repeated courses can be implemented with an interval of one month. Alternatively, the medication can be used by 450 mg split into three intakes and taken for two-three weeks with an interval of 2-3 months for one to two years.
- N. meningitides carriage requires the use of the medication for four days. The daily dosage for adults is 600 mg, for children it is 10-12 mg per kilogram of the body weight.

Dosage adjustments

In liver function impairment, the daily dosage must not exceed 8 mg per kilogram of the body weight.
- The daily dosage for elderly patients is chosen based on their kidney function. If the kidney function is impaired, the dosages are lowered respectively as the liver takes the part of the work if kidneys are not managing the excretion. It is not advised to use the medication in senior patients if they have liver dysfunction.

Rifadin overdose

The symptoms of an overdose include nausea, vomiting, pain in the abdomen, fatigue, sleepiness, allergic reactions, fever, wheezing, jaundice, rapid heart rate, low blood pressure, and others.
The treatment is made of discontinuation of the formulation use, gastric lavage during the first two-three hours after an overdose, intake of activated carbon, and symptomatic therapy. In severe cases, forced diuresis and hemodialysis can be performed.

Rifadin adverse effects

Rifadin interactions

The formulation is a potent inducer of P450 cytochrome which can cause potentially harmful interactions. Thus, before you order Rifadin, inform your doctor about all other medications that you use.
For instance, it is contraindicated to use it along with Saquinavir/Ritonavir.
Rifadin diminishes the effect of indirect anticoagulants, corticosteroids, oral hypoglycemic medicines, antiarrhythmic medications, benzodiazepines, oral contraceptives, and other hormones, and others (see the full list in the package insert).
Antacids prevent the formulation of normal absorption and thus make it less effective.
In the simultaneous use with opiates, anticholinergics, and ketoconazole, the bioavailability of the medication substantially drops.
The level of the medication in the blood plasma is elevated in the same time use with Probenecid and co-trimoxazole.
The likelihood of severe liver dysfunctions rises in the simultaneous use of the antibiotic with Isoniazid or Pyrazinamide.

Rifadin precautions

For the non-tuberculosis diseases, the antibiotic is appointed only if the first choice antibiotics are ineffective.
Individuals with kidney dysfunction require the dosage adjustments if the dosage is above 600 mg a day.
The antibiotic must be cautiously used in newborn or prematurely born babies and in weakened patients. Newborns need also to be given vitamin K as a prophylaxis of bleedings.
Rifadin must be cautiously used in HIV patients.
In flu-like syndrome development during the intermittent therapy, it is recommended to shift to the daily medication intake with the gradual dosage increase.
During the therapy, regular tests of the blood, liver and kidney function must be made.
In case the medication is appointed for the carriers of Neisseria meningitides, it is necessary to perform a strict observance over the patient to timely notice the symptoms occurrence if the resistance develops.
The formulations that lower the absorption of the medication (for instance, antacids) must be taken no sooner than four hours after Rifadin.
During the therapy, it is encouraged to refrain from alcohol use to minimize the drug’s negative effect on the liver.
