Acarbose is a pseudotetrasaccharide of microbial origin. It acts in the digestive tract, inhibiting the activity of small bowel enzymes, which are involved in the disintegration of di-, oligo- and polysaccharides. As a result, there is a dose-dependent delay in the capture of carbohydrates, followed by an inhibited-release and absorption of glucose, which is formed during the metabolism of carbohydrates. Thus, Acarbose delays and reduces the after-meal rise in blood sugar. Due to a more balanced absorption of glucose from the bowel, the average concentration and its daily fluctuations in the blood are reduced. The use of the medication is indicated in:

- Type 2 diabetes with the ineffectiveness of diet therapy (the course of which should be at least 6 months) or the insufficient effectiveness of sulfonylurea derivatives along with the following of a low-calorie diet;

- Type 1 diabetes mellitus (as part of complex therapy);

- Prophylaxis of type 2 diabetes (in individuals with impaired glucose tolerance along with diet and physical exercise).

When the use of Precose is prohibited?

- Hypersensitivity to Acarbose and/or excipients.

Acarbose dosage regimens

The optimal dosage is set individually for each patient, since the effectiveness and tolerability of the medication may vary.

Pills are taken without chewing, with a small amount of liquid, immediately before meals or with the first serving of food.

Combination therapy with diet

The primary dosage is 150 mg split into three intakes. Then, if necessary, the dose can be raised to 100 mg 3 times a day. In some cases, if necessary, it can be even raised to 600 mg split into three intakes.

The elevation of the dosage must be made only after 4-8 weeks if the necessary clinical effect was not obtained with the previous dose. If the individual has complaints of well-being, despite strict following of the appointed diet, then a further rise in the dosage must be stopped, and if needed, even slightly reduced. The average daily dosage is 300 mg per day.

Prevention of type II diabetes in individuals with confirmed impaired glucose tolerance with diet and exercise

The regular dosage is 300 mg split into three intakes during the day.

The dosage in the beginning is 50 mg once a day. Within 3 months, it can be raised to 300 mg split into three intakes.

Elderly patients (over 65): dose adjustment is not required.

Individuals with liver insufficiency do not require dose adjustment.

Overdose of Acarbose

If a too high dose is used simultaneously with drinks and/or food products containing carbohydrates, an overdose may be accompanied by bloating and diarrhea. In cases of overdose, it is advised that drinks and dishes with high content of carbohydrates be excluded from the diet for 4-6 hours. In the event of an overdose without food, no development of symptoms from the intestine is expected.

Precose adverse reactions

- Thiazides, corticosteroids, thyroid hormones, estrogens, including as part of oral birth control, Phenytoin, nicotinic acidcalcium antagonists, Isoniazid and other agents that cause hyperglycemia, significantly weaken the specific activity (decompensation of diabetes is possible).

Special recommendations

- It is advised that the level of liver enzymes be monitored during the first half of a year or a whole year of treatment since when using the medication, an asymptomatic increase of their levels possible. As a rule, after discontinuation of the course, their activity is normalized.

- The action and harmlessness of Acarbose in individuals under the age of 18 has not been sufficiently studied.
