  Generic Medication guide:

What is Precose and when is it indicated to be used?

Precose is a brand owned by the German pharmaceutical concern Bayer. The product sold under this name is based on the substance with an unpatented name Acarbose. Since the exclusive patent of the developer of the substance expired, it can be produced by other pharma companies, for instance, by Glenmark Pharmaceuticals. Acarbose is a pseudotetrasaccharide of microbial origin. It acts in the digestive tract, inhibiting the activity of small bowel enzymes, which are involved in the disintegration of di-, oligo- and polysaccharides. As a result, there is a dose-dependent delay in the capture of carbohydrates, followed by an inhibited-release and absorption of glucose, which is formed during the metabolism of carbohydrates. Thus, Acarbose delays and reduces the after-meal rise in blood sugar. Due to a more balanced absorption of glucose from the bowel, the average concentration and its daily fluctuations in the blood are reduced. In the case of an increase in the concentration of glycosylated hemoglobin, Acarbose reduces its level too.

The use of the medication is indicated in:

- Type 2 diabetes with the ineffectiveness of diet therapy (the course of which should be at least 6 months) or the insufficient effectiveness of sulfonylurea derivatives along with the following of a low-calorie diet;

- Type 1 diabetes mellitus (as part of complex therapy);

- Prophylaxis of type 2 diabetes (in individuals with impaired glucose tolerance along with diet and physical exercise).

When the use of Precose is prohibited?

You should not buy generic Precose and use it even if you have the indications for its use in case of:

- Hypersensitivity to Acarbose and/or excipients.

- Chronic bowel diseases with severe digestive and absorption disorders including ulcerative colitis, intestinal obstruction, partial or predisposition to it.

- Conditions accompanied by flatulence (Roemheld syndrome, large hernias, stenosis, and intestinal ulcers).

- Severe renal failure (creatinine clearance<25 ml / min).

In case of fever, infections, injuries, surgical interventions (forthcoming surgery and in the postoperative period), the use of the medication is possible only if the expected effect exceeds the potential risk.

In age under 18 years (safety and efficacy not determined).

Acarbose dosage regimens

The optimal dosage is set individually for each patient, since the effectiveness and tolerability of the medication may vary.

Pills are taken without chewing, with a small amount of liquid, immediately before meals or with the first serving of food.

The duration of therapy is not limited.

Combination therapy with diet

The primary dosage is 150 mg split into three intakes. Then, if necessary, the dose can be raised to 100 mg 3 times a day. In some cases, if necessary, it can be even raised to 600 mg split into three intakes.

The elevation of the dosage must be made only after 4-8 weeks if the necessary clinical effect was not obtained with the previous dose. If the individual has complaints of well-being, despite strict following of the appointed diet, then a further rise in the dosage must be stopped, and if needed, even slightly reduced. The average daily dosage is 300 mg per day.

Prevention of type II diabetes in individuals with confirmed impaired glucose tolerance with diet and exercise

The regular dosage is 300 mg split into three intakes during the day.

The dosage in the beginning is 50 mg once a day. Within 3 months, it can be raised to 300 mg split into three intakes.

Elderly patients (over 65): dose adjustment is not required.

Individuals with liver insufficiency do not require dose adjustment.

Overdose of Acarbose

If a too high dose is used simultaneously with drinks and/or food products containing carbohydrates, an overdose may be accompanied by bloating and diarrhea. In cases of overdose, it is advised that drinks and dishes with high content of carbohydrates be excluded from the diet for 4-6 hours. In the event of an overdose without food, no development of symptoms from the intestine is expected.

Precose adverse reactions

If you buy Acarbose for the first time, you should know that it can cause such negative effects as:

- Digestive tract: abdominal pain, bloating, nausea, diarrhea; rarely – a raise in the level of transaminases, intestinal obstruction, jaundice, hepatitis. - Allergy: skin rash, hyperemia, exanthema, and urticaria.

- Other: rarely – swelling.

Acarbose interactions

- If the formulation used together with other antidiabetic substances, the blood sugar level may drop to the hypoglycemic range (in rare cases, hypoglycemic coma may develop), which requires a reduction in their dose.

- In rare cases, Acarbose can affect the bioavailability of Digoxin, which entails the need to adjust the dose of the latter.

- It is not recommended to combine the medication with cholestyramine, activated carbon, digestive enzyme medications containing pancreatin or amylase, intestinal sorbents due to their possible lowering effect on the effectiveness of Acarbose.

- Thiazides, corticosteroids, thyroid hormones, estrogens, including as part of oral birth control, Phenytoin, nicotinic acidcalcium antagonists, Isoniazid and other agents that cause hyperglycemia, significantly weaken the specific activity (decompensation of diabetes is possible).

Special recommendations

- It is advised that the level of liver enzymes be monitored during the first half of a year or a whole year of treatment since when using the medication, an asymptomatic increase of their levels possible. As a rule, after discontinuation of the course, their activity is normalized.

- The action and harmlessness of Acarbose in individuals under the age of 18 has not been sufficiently studied.

- In case of non-compliance with the diet appointed by the doctor, adverse reactions from the digestive tract may intensify. If the symptoms worsen, despite the strict observance of the diet, it is necessary to consult a doctor and reduce the dose for a short or long term.

- The use of sucrose (food sugar) and products containing it can cause gastrointestinal discomfort and diarrhea due to increased fermentation of carbohydrates in the bowel.

- The medication exhibits a hypoglycemic effect but does not induce a drop of blood sugar below the normal range.

- During the use of the medication, sucrose (food sugar) is more slowly broken down into fructose and glucose, therefore it is unsuitable for the quick correction of acute hypoglycemia. In this case, glucose should be used.
