What is Aciphex and when is it prescribed?

The medication known as Aciphex is made and marketed by the company Janssen-Cilag. The same medication is made and sold also by other companies, for instance, Indian generics-maker Lupin. The active substance of the medication which ensures its effect has an unpatented name Rabeprazole.

Rabeprazole is a prodrug - in the acidic environment, it turns into an active sulfenamide form that interacts with cysteine which acts as a proton pump inhibitor, i.e. lowers the production o gastric acid. The effect is manifested within one hour after ingestion of a dose of 20 mg. The maximum decrease in the pH of the stomach is recorded two to four hours after the first dosage use. On the first day, it decreases the average daily acidity by 61% (this is about 88% of the drop in secretion achieved on the 8th day of the course).

It has anti-Helicobacter pylori activity as it accelerates the anti-Helicobacter pylori activity of a number of antibiotics. The effect of the therapy of erosive reflux esophagitis is observed in 84% cases.

- For the therapy of acute duodenal ulcer and benign gastric ulcer, the regular dosage is 20 mg once a day before breakfast. Usually, the time required to heal the ulcer is up to one month. However, it should be noted that some patients should take the medication for an additional 4 weeks to recover. In most cases, healing takes place within 6 weeks, but for some patients who are not sensitive to treatment, an additional 6 weeks are needed.

- Erosive or peptic ulcer and GERD are treated with 20 mg 1 time per day for one to two months.

- Maintenance therapy of GERD is made with the doses of 10 mg or 20 mg once a day. The dosage is chosen based on the response to the therapy.

- Therapy of GERD from moderate to very severe: patients without esophagitis should be prescribed 10 mg once a day. If after a month of the course the symptoms do not disappear, an additional examination is carried out. As soon as the symptoms are eliminated, further control of the symptoms is made with 10 mg once a day only when needed, for example, if you plan to eat food that provokes heartburn.

- Zollinger-Ellison syndrome: the dosage is chosen individually based on the symptoms and response to the therapy. The initial dosage is usually 60 mg per day. The dose can be gradually raised to 120 mg per day in clinical need. If necessary, take 120 mg per day, divide it into two doses of 60 mg. Some individuals with the disease need not less than a year of the uninterrupted course.

- H. pylori: individuals with H. pylori should use appropriate combinations of the medication with antibiotics. The recommended regimen implies the use of the medication 20 mg 2 times a day + Clarithromycin 500 mg 2 times a day and amoxicillin 1 g 2 times a day for seven days.

Impaired kidney and liver function

- Individuals with impaired kidney and liver function do not require dose adjustment.

- The use for the treatment of individuals with severely impaired liver function is discussed in more detail in the section “Special recommendations”.

Adverse reactions

The most frequetn adverse reactions recorder are headache, diarrhea, abdominal pain, bloating, rashes, and dry mouth. -Vascular disorders: peripheral edema, high arterial tension, irregular heartbeat.

- Respiratory system: cough, sore throat, nasal congestion.

- Digestive tract: diarrhea, nausea, vomiting, constipation, bloating, feeling of fullness and heaviness in the stomach, candidiasis, dry mouth, belching, gastritis, stomatitis, violation of taste, enteritis, esophagitis, heartburn, hemorrhoids.

- Disorders of the liver and biliary tract: hepatitis, jaundice. Caution should be exercised when using the treatment for individuals with severe liver dysfunction.

Overdose

Interactions with other substances

The concentrations of Rabeprazole and the active metabolite of clarithromycin in plasma with simultaneous administration increase by 24 and 50%, respectively. This increases the effectiveness of this combination in the eradication of Helicobacter pylori. The study found no interaction of Rabeprazole with liquid antacids. No clinically significant interaction of Rabeprazole with food has been identified.

Special recommendations

- Senior individuals. The formulation in the liver and since sometimes the function of the organ is impaired in older age, such individuals require observation and strict adherence to recommendations on dosage and duration of treatment.

- The improvement of symptoms can also occur in the presence of cancerous neoplasms in the stomach, therefore, before starting therapy, it is crucial to exclude the presence of tumors.

- Long-term treatment course requires frequent monitoring.

- Deviations of liver enzymes from the norm were observed. In most cases, the violations were uncomplicated and disappeared upon the cessation of the course.

- In mild or moderate impairment of liver function, no significant difference in the incidence and severity of adverse effects is recorder. However, the physician should be careful when prescribing the course to individuals with severe liver failure as the safety in this group isn’t sufficiently studied.

- The course with the medications from the class is associated with elevated risk of digestive tract infections such as Salmonella, Campylobacter, and others.

- Hypomagnesemia (symptomatic or asymptomatic) can occur in individuals who use the medication for three months or more, and may manifest with symptoms such as arrhythmia, convulsions, and others. In the occurrence of symptoms, the cessation of the therapy and corrective therapy with magnesium is required.

- It is recommended that thyroid function be monitored throughout the course as it can be affected by the medication.

- As a supportive therapy for reflux esophagitis, the medication can be prescribed only to indivduals with relapses of the disease. It should not be appointed to those who do not need maintenance therapy. It is advised to discontinue treatment when improving health status through diet, limiting the use of alcohol, and also in the absence of flare-ups for a long time. A thorough examination, namely periodic endoscopy, indicated with maintenance therapy.

- The course for non-erosive reflux disease is indicated in recurring reflux symptoms, in particular heartburn and acid reflux (about twice a week).

- The doctor should make sure that the treatment is effective after two weeks of using. If the symptoms do not fade, it is likely that reflux disease is not the cause of their occurrence. In this case, the doctor should apply a different treatment method.
