Pharmacological properties
Diclofenac belongs to the group of NSAIDs. It has a pronounced anti-inflammatory, as well as moderate antipyretic activity. The mechanism of action is due to inhibition of mediators of inflammation synthesis. By inhibiting the activity of the COX-1 enzyme, it reduces platelet aggregation. The formulation is effective in the therapy of rheumatoid conditions and inflammation due to injury. It diminishes pain, liquidates inflammation and edema with rheumatic or injury-induced inflammation, and betters the function of the muscular system. In the course of therapy of rheumatoid arthritis, the medication intensively penetrates the joint cavity, reduces pain at rest and during movement, helps to elevate the range of movements in the affected joints, and reduces morning rigidity and puffiness of the joints. A stable effect is observed within one to two weeks of treatment.
Pharmacokinetics
When applied to the skin, it is partially absorbed (3.3% of the locally used dose) and detected in blood plasma. The formulation penetrates well into tissues and joints fluid, where its level is higher than in the blood plasma. T1⁄2 from blood plasma is 1–2 hours, from synovial fluid - 3–6 hours.

Application
The drug is used externally in adults and kids over the age of 12 years. The dosage of the gel depends on the size of the affected area. Usually, 2–4 g of gel (a size of pea or walnut) is applied to the skin at the area in pain 3-4 times a day and is gently rubbed in. Special Instructions
- Use during pregnancy and lactation. The use of the formulation is prohibited in the III trimester of pregnancy due to the possibility of developing weakness in the labor and/or premature closure of the ductus arteriosus, in the I and II trimester the use of the gel is possible only as prescribed by the healthcare provider in the case when the expected benefit to the mother outweighs the potential risk to the fetus. Animal studies have not revealed a single manifestation of the harmful effects of Diclofenac on pregnancy or fetal development, childbirth or postnatal development.
- Diclofenac sodium passes into breast milk in an insignificant amount; therefore, if there are substantial reasons for using the gel during breastfeeding, it should not be applied to the mammary glands or large areas of the skin and applied for a long time.
