Pharmacokinetics
When administered orally, it is quickly and fully absorbed from the digestive tract. The highest plasma level is reached within three to five hours. The equilibrium concentration in the blood is established for 7-12 days. It is distributed in all tissues and organs. It binds to plasma proteins up to 90–98%. When used simultaneously with other drugs, it can displace them when bound to proteins, as a result of which they can enhance their therapeutic effect.
It penetrates through the placental and blood-brain barrier and is not cumulated.
The half-life varies and is around 50 hours. It is lengthened in liver diseases. It is secreted mainly by the kidneys and through the digestive tract (2 times more is found in urine than in feces), mainly in the form of glucuronides. It penetrates into breast milk.
When can be Feldene used?

Interactions of Feldene

- The formulation must not be combined with other NSAIDs or aspirin as it doesn’t help with the therapy but can only potentiate the adverse effects on the organism. - The use along with corticosteroids results in high risk of digestive tract ulceration or bleeding.
- The medication enhances the effect of blood thinners and cause bleeding so their use must be avoided.
- Methotrexate: Feldene lowers the washing out of Methotrexate, which can lead to acute toxicity.
- Use with Tacrolimus results in elevated risk of toxic impact on the kidneys.
- Use with antiplatelet agents and SSRI antidepressants results in an elevated risk of bleeding in the digestive tract.
- Lithium: Piroxicam can elevate the concentration of lithium salts in blood plasma and lengthen, enhance their effect.
- The medication lowers the effect of diuretics and they raise the toxic effect of Feldene.
- In the simultaneous use of Piroxicam with potassium-sparing diuretics or other formulations containing potassium, there is a risk of hyperkalemia.
- Blood tension medications: Piroxicam may lower the effect these medicines when used simultaneously.
-With cardiac glycosides, it can cause flare-up of heart failure, drop in kidney filtration function, and elevated plasma level of cardiac glycosides.
-Quinolones: with the simultaneous use of quinolones and piroxicam, the risk of seizures in patients with a history of epilepsy or seizures increases.
-Aminoglycosides - simultaneous use with aminoglycosides in individuals with kidney dysfunction leads to a drop in excretion and an elevation in the plasma level of the latter. - Probenecid diminishes the metabolism and elimination of NSAIDs and their metabolites with simultaneous use.
- Antidiabetic oral drugs: NSAIDs inhibit the metabolism of sulfa medicines and raise the risk of hypoglycemia.
- Antacids: the simultaneous use of antacids does not affect the level of piroxicam in blood plasma.
- Mifepristone: NSAIDs may affect Mifepristone-mediated abortion.
- Phenytoin: an increase in the level of Phenytoin in the blood is possible, appropriate control and dose lowering are recommended, if Piroxicam therapy is started, if necessary, discontinue therapy.
- Cyclophosphamide, vinca alkaloids: taking Piroxicam before or after therapy with these drugs may elevate the adverse reactions of these substances (combinations should be avoided).
- Cyclosporine: raised risk of digestive tract damage, damage to the kidneys and/or liver.
- With Alcohol, impaired medication tolerance is observed.
- Drugs that are largely associated with blood plasma proteins: Piroxicam is associated largely with proteins, which may displace other protein-related drugs.
When applying Piroxicam to patients who take other drugs, which are largely associated with proteins, doctors should carefully monitor the condition of patients and, if necessary, adjust the dose.
Dosages and course duration with Feldene

The probability of adverse reactions can be lowered by using the minimum effective dosage for the shortest period.

Adults

In the combined use of different forms of the medication (capsules, tablets, solutions, suspensions, suppositories), the total daily dose should not exceed 20 mg.
Elderly patients (over 65 years old)
For individuals over 65 years of age, special care is required, especially for in impaired renal, liver, or heart function.
Overdose
Symptoms of an overdose Gastrointestinal disorders such as nausea, abdominal pain, possibly bleeding in the digestive tract, weakness, headache, confusion, tinnitus and hyperventilation with respiratory alkalosis, arterial hypertension. At later stages, central nervous system depression, hyperthermia, respiratory and metabolic acidosis, toxic circulatory failure, impaired kidney and liver function, cerebral and pulmonary edema may develop; increased likelihood of seizures and coma.
Overdose treatment
It is necessary to consider the long half-life of Piroxicam. Based on animal tests, it can be assumed that the washing out of Piroxicam is accelerated by the use of antacids and activated carbon.
- Primary elimination of the drug (careful gastric lavage);
- Monitoring of acid-base balance;
- Electrolyte correction, glucose correction;
- Intensive medical care;
- Acceleration of the elimination process (alkalized, forced diuresis);
- Diazepam administration in case of seizures.
- The introduction of activated carbon affects the resorption and absorption of Piroxicam and, thus, reduces the total amount of the active agent in blood serum
