The effect of the medication is due to binding to adenosine receptors. It also blocks phosphodiesterase, stabilizes cAMP and lowers the level of intracellular calcium. Theophylline relaxes the smooth muscles of internal organs (bronchi, digestive system and uterus), suppresses mast cell degranulation and reduces the level of allergy mediators (serotonin, histamine, etc.) in the blood (anti-allergic effect). Increases the tone of the respiratory muscles (intercostal muscles and diaphragm); dilates blood vessels of the lungs and improves blood oxygenation; inhibits platelet aggregation, enhances the resistance of red blood cells to deformation, suppresses thrombosis and normalizes microcirculation. The antianginal effect (increased oxygen delivery to the myocardium) is due to the expansion of the coronary arteries.
After oral administration, it is completely absorbed (bioavailability over 90%). Cmax in the blood is reached within 2 hours (normal form) and 6 hours (prolonged form). In blood, 40-60% bound to proteins. Penetrates through blood-tissue barriers. It undergoes biotransformation in the liver with the formation of inactive metabolites. The formulation is washed out mainly by the kidneys (up to 13% in adults and up to 50% in children - unchanged); partially it is washed out in breast milk.

How is Theo-24 used?

Interaction with other medicines
- Toxic synergism with ephedrine was noted; this can also happen with other bronchodilators sympathomimetics.
- Allopurinol, Phenobarbital, cimetidine, norfloxacin, ciprofloxacin, erythromycin, oral contraceptives, and Propranolol contribute to an increase in Theophylline concentration in blood plasma.
- Phenytoin, Methotrexate, and Rifampicin reduce Theophylline concentration.

Special recommendations

- Smoking significantly affects the metabolism and elimination of Theophylline. In individuals who smoke more than 20 cigarettes per day, the elimination half-life is reduced.
- Excretion of the active substance is increased in people with severe respiratory failure, with liver and kidney failure, viral infection, hyperthermia.
- Alcohol and caffeinated foods also affect Theophylline metabolism.
