What is Niacin and when is it used?

Niacin is one of the brand under which the compound with an unpatented international name nicotinic acid is sold. You can buy nicotinic acid under other less known names to get it cheaper.

The mediation has hypocholesterolemic, hypolipidemic, vasodilating, replenishing vitamin PP deficiency (B3) effects. It is included in the group of enzymes that transport hydrogen. Niacin regulates redox processes (oxidation and restoration), tissue respiration, protein and fat synthesis, glycogen breakdown, inhibits lipolysis in adipose tissue, reduces the rate of synthesis of very low density lipoproteins (bad cholesterol), and normalizes the lipid composition of the blood (reduces the level of total cholesterol, bad cholesterol, triglycerides and increases the level of good cholesterol).

Nicotinic acid dilates blood vessels including the vessels of the brain, improves microcirculation, and prevents the formation of thrombus. Nicotinic acid is also found in foodstuffs but if the food rich in it is consumed insufficiently and the disorders related to it develop, it is advised to use this vitamin in pills.

When is the use of Niacin contraindicated?

- For other diseases, adults should use 50mg twice a day. Kids older than 12 years are given 25 mg twice a day.

- The highest dosages for adults – 100 mg single dosage and 500 mg total daily dosage. In atherosclerosis (in the absence of side effects), a single dose can be gradually increased to 500 mg-1 g, and the daily dose to 2-3 g.

- The highest dosage for kids is 50 mg single dosage and 200 mg daily dosage.

Nicotinic acid overdose

Symptoms include dilation of the peripheral vessels resulting in skin reddening, nausea, vomiting, diarrhea, heartburn, brown spots on the skin, dizziness, jaundice of the skin, skin paresthesia (tingling, burning sensation, sensation of heat, crawling sensation).

In the occurrence of these symptoms, it is required to stop using the medication and undertake measures to eliminate the symptoms.

What adverse effects can Niacin cause?

When you buy nicotinic acid, you should know that it can cause such adverse effects as:

- Cardiovascular system: flushes that can be accompanied by shortness of breath, irregular or rapid heartbeat, sweating, chills, swelling.

- Liver and bile ducts: in long-term use - fatty liver, increased blood levels of aspartate aminotransferase (ATS), lactate dehydrogenase (LDH), alkaline phosphatase.

- Metabolism: in long-term use in high doses – impaired tolerance of glucose, increased glucose levels, increase of uric acid levels in the blood, hypophosphatemia.

- Other: allergic reactions including urticaria. Very rarely the use of the medication in high doses in individuals with cardiac ischemia can increase the frequency of atrial arrhythmias, as well as acanthosis (brown spots on the skin with a velvety surface).

Rarely, edema of the retina may occur. These symptoms are unstable and disappear after discontinuation of the drug.

The use of high doses of nicotinic acid can cause: hyperpigmentation, seizures, diarrhea, nausea, vomiting, anorexia, exacerbation of stomach ulcers, jaundice, amblyopia, decreased platelet count, prolonged prothrombin time.

Insomnia, myalgia, arterial hypotension, and rhinitis may occur. Possible blurred vision, swelling of the eyelids, myopathy, exfoliative dermatitis.

Cases of rhabdomyolysis when using nicotinic acid with Lovastatin have been reported.

Can Niacin be used with other medicines?

- It is not recommended to use Lovastatin and/or Pravastatin with nicotinic acid as there is a risk of enhanced adverse reactions up to rhabdomyolysis (muscle cells destruction).

- The medication should be cautiously used with blood tension lowering medications as it can enhance their effect.

- Niacin should be cautiously used with anticoagulants (blood thinners) and aspirin since there is a risk of internal bleeding.

- The medication potentiates the effect of fibrinolytic agents, antispasmodics and cardiac glycosides, and the toxic effect of alcohol on the liver.

- The simultaneous use with beta-adrenergic receptors blockers or with methyldopa leads to a strong decrease in blood pressure; with antidiabetic agents - to the potentiating of the glucose-lowering effect of the latter.

Special recommendations for nicotinic acid use

- Control the level of glucose and liver function to timely notice if there are negative effects of the drug on these systems.

- It should be carefully used by people with history of internal bleedings (including eye hemorrhages), glaucoma, kidney failure, moderate arterial hypotension, high acidity gastritis, peptic ulcer of the stomach and duodenum (in remission), alcohol abusers, patients with unstable angina pectoris, and acute myocardial infarction receiving nitrates, calcium channel antagonists, and beta blockers.

- In the long-term use in high dosages, the medication can cause fatty liver which is why the control over the liver function is needed. To prevent liver complications, it is recommended to include foods rich in methionine (cottage cheese) in the diet or to use methionine and other lipotropic drugs.

- It is needed to regularly monitor the level of uric acid in the blood due to its possible increase as a result of prolonged therapy.

- The pills contain glucose which should be considered by people with diabetes. - It is not advised to use nicotinic acid for the correction of dyslipidemia in patients with diabetes.
