Indications for Zyloprim use:

Treatment for all forms of hyperuricemia not controlled by diet, with serum uric acid levels in the range of 9 mg/100 ml;

- Diseases caused by an elevated level of uric acid in the blood serum, in particular gout, urate nephropathy and urate urolithiasis; - Secondary hyperuricemia of various origins;

- Primary and secondary hyperuricemia in various hemoblastoses (acute leukemia, chronic myelogenous leukemia, lymphosarcoma).

100 mg tablets are used for kids and adolescents with the body mass over 15 kilograms for:

- Secondary hyperuricemia;

- Urate nephropathy resulting from the treatment of leukemia;

- Congenital enzymatic deficiency and deficiency of adenine phosphoribosyl transferase.

300 mg tablets are used for kids with the body mass over 46 kilograms and adults for:

- Secondary hyperuricemia;

- Urate nephropathy resulting from the treatment of leukemia;

- Congenital enzymatic deficiency and deficiency of adenine phosphoribosyl transferase.

The active substance disrupts the synthesis of uric acid, possess urostatic properties, which are mainly based on the ability to suppress the enzyme which catalyzes the formation of uric acid. The medication leads to a diminished level of uric acid and helps to dissolve urates (uric acid salts).

When is the use of Zyloprim contraindicated?

You should not buy Zyloprim and use it in:

- Allergy or high sensitivity of the medication's ingredients;

- Severe kidney dysfunction. - Severe liver dysfunction.

With creatinine clearance<20 ml/min, 300 mg tablets are not used.

How is Zyloprim used?

When you buy Allopurinol, you should consult your doctor about the dosages as it should be set individually depending on the level of uric acid level. To lower the risk of negative reactions, the course is begun with 100 mg once a day. The dose is elevated only if the level of uric acid isn’t sufficiently lowered with this dosage. The pills are taken after a meal with a full glass of water.

The length of the course depends on condition progression. For the prevention of the formation of oxalate and urate stones, in primary hyperuricemia, and gout, in most cases, long-term therapy is necessary. In secondary hyperuricemia, the short-term course is recommended.

Regular dosage regimens:

- In mild stage - from 100 to 200 milligrams a day;

- In moderately severe stage - from 300 to 600 milligrams a day;

- In severe stage - from 700 to 900 milligrams a day.

If the prescribed daily dose is more than 300 mg, it should be split into several intakes.

When the dosage is calculated based on weight, it is 2-10 mg per kilogram of the body weight for a day.

- Children and teens with the body weight ≥15 kg.

The daily dose of is 10 mg/kg, divided into three doses. The maximum daily dose is 400 mg. Use 100 mg tablets.

- Children and teens with the body weight ≥45 kg.

The daily dose of allopurinol is 10 mg/kg, split into three doses. The highest daily dose is 400 mg.

- Elderly patients. Due to the lack of special data on the use of Zyloprim in this category of patients, it is advised to use it in the lowest therapeutically reasonable doses because there is a risk of impaired kidney function.

- Individuals with kidney failure. Since the medication is eliminated by the kidneys, if their function is impaired, an overdose is possible if the dose has not been properly selected. In severe kidney impairment, the highest daily dose is 100 mg. It is possible to use a single dose of 100 mg with an interval of more than a day (every 2-3 days).

If it is needed to increase the dose, it is necessary to control the level of oxypurinol in blood plasma, which should not exceed 15.2 micrograms per milliliter.

Creatinine clearance, ml/min Daily dosage
>20 Standard dosage
10–20 100–200 mg
<10 100 mg or 100 mg every 2–3 days

For patients on hemodialysis, the dosage of 300-400 mg is taken after each dialysis procedure (2-e times a week).

- For individuals with liver dysfunction, the medication should be appointed in lower dosages. Besides, at the beginning of the therapy, their liver function should be regularly controlled. In liver dysfunction, do not use 300 mg pills.

Zyloprim overdose

In the case of a single time overdose, in the majority of cases, no negative symptoms develop. But in the prolonged overdose in impaired kidney function, such symptoms as skin allergy, fever, hepatitis, kidney failure aggravation, and others can develop. Thus, it is very important to follow your doctor’s recommendations on the dosages and avoid overdosing. In suspected acute overdose, it is advised to do gastric lavage, induce vomiting, and take sorbents (for instance, activated carbon) within an hour of a high dosage intake.

What adverse effects can Zyloprim cause?

When you buy generic Allopurinol, you can develop certain negative effects. They are more likely to occur in kidney or liver failure or in a combination of the the course use with ampicillin or amoxicillin. The possible negative effects are:

- At the beginning of the therapy, the exacerbation of gout can occur.

- On the part of the skin: Stevens-Johnson syndrome, alopecia, furunculosis, Quincke's edema, hair bleaching. The most common skin reactions (≈4%) occur at any period of treatment and can manifest themselves as purple-like, maculopapular, squamous, or exfoliative rashes. If any skin reactions occur, the course should be ceased immediately. After reducing the severity of symptoms, the medication's use can be resumed in low doses, for example, 50 mg a day. If necessary, this dose can be gradually raised. With a repeated occurrence of a skin rash, the use of the medication is ceased, as severe generalized allergic reactions may appear.

- On the part of the immune system: delayed-type hypersensitivity reactions, accompanied by fever, skin rash, pseudolymphoma, arthralgia, leukopenia, eosinophilia, hepatosplenomegaly, reversible increase in transaminases and alkaline phosphatase of the liver; acute cholangitis and xanthine stones; anaphylactic shock. On the part of the liver: impaired liver function from an asymptomatic increase in liver function indices to hepatitis (including liver necrosis and granulomatous hepatitis).

- Digestive tract: nausea, vomiting, diarrhea; hemathemesis, steatorrhea, stomatitis.

- Circulatory system: severe bone marrow damage (thrombocytopenia, agranulocytosis, aplastic anemia), especially in patients with renal failure; changes in blood counts (leukopenia, leukocytosis, granulocytosis and eosinophilia), true red blood cell aplasia.

- Respiratory system, chest and mediastinal organs: angina pectoris.

- From the nervous system: ataxia, peripheral neuritis, violation of taste sensations, coma, headache, neuropathy, paralysis, dizziness, drowsiness, paresthesia.

- From the cardiovascular system: bradycardia, hypertension.

- Metabolic disorders: diabetes mellitus, hyperlipidemia.

- Psyche: depression.

- Reproductive system: gynecomastia, impotence, infertility.

- Urinary system: hematuria, uremia.

- Vision: cataract, retinal degeneration, visual impairment.

- Musculoskeletal system: muscle pain.

- General disorders: general malaise, asthenia, edema.

Can Zyloprim be used with other drugs?

Before you order Zyloprim, make sure to check with your doctor that you don’t take any medications it is incompatible with:

- The use of the medication with mercaptopurine or Azathioprine requires the lowering of the latter should be lowered by 25%.

- The medication should be cautiously used with vidarabine to avoid a possible increase in toxic effects.

- The effectiveness of allopurinol may decrease with the simultaneous use with medicines that are able to excrete uric acid (sulfinpyrazone, probenecid, benzbromarone or salicylates).

- Allopurinol slows the elimination of probenecid.

- In case of impaired kidney function, especially when used simultaneously with allopurinol, the hypoglycemic effect of chlorpropamide may be prolonged, which requires a dose reduction.

- The effect of warfarin and other blood thinners - coumarin derivatives can intensify, therefore, frequent monitoring of blood coagulation, as well as a lowering in the dose of anticoagulants, is necessary.

- In high doses, allopurinol inhibits metabolism and raises the level of theophylline and caffeine in blood plasma. At the beginning of therapy with Zyloprim or with a raise in its dose, it is required to control the Theophylline level in blood plasma.

- The simultaneous use with Ampicillin or amoxicillin enhances the likelihood of skin allergic reactions (exanthema), so other antibiotics should be used.

- When using Zyloprim with cytostatics, increased inhibition of bone marrow function was noted, therefore, blood counts should be checked at short intervals.

- When used with Zyloprim, the level of cyclosporine in blood plasma may raise and its toxicity may increase. - Didanosine simultaneous use with allopurinol is not advised.

- The simultaneous use of the medication and Captopril can raise the risk of skin reactions, especially in chronic kidney diseases.

Special recommendations for Zyloprim

- If during the use of the formulation allergy occurs, the use of Zyloprim should be ceased. It can be resumed later with low doses unless severe allergic reactions occurred (you doctor should assess their severity).

- The medication use is not advised if the level of uric acid in the blood plasma is<535 mol/L (9 mg/100 ml), subject to recommendations regarding diet and the absence of kidney damage. Do not eat foods high in purine (for example, offal: kidneys, brain, liver, heart and tongue; meat bacon and alcohol, especially beer).

- In kidney or liver dysfunction or previously identified violations of hematopoiesis, careful medical supervision is needed. Besides, the lowering of the dosage is advised.

- Individuals with elevated arterial tension or heart failure using ACE inhibitors or diuretics should use the medication with extreme caution.

- In the treatment of gout and urolithiasis, the volume of urine excreted should be at least 2 l/day. To prevent a rise in the level of uric acid in blood plasma or urine, which can occur with radiotherapy or chemotherapy of neoplasms, as well as with Lesch-Nyhan syndrome. large amount of liquid should be used to support normal urine output. Besides, in order to improve uric acid excretion, alkalization of the urine can be resorted to, which leads to the dissolution of urate / uric acid.

- If urate nephropathy or other pathological changes have already caused kidney dysfunction, the dose should be corrected according to indicators of renal function.

- If there are bouts of acute gout, therapy should not be started until they disappear completely. At the beginning of treatment with Zyloprim, acute attacks of gout may enhance as a result of the movement of a large amount of uric acid. Throughout the first months of therapy, the simultaneous use of analgesics or colchicine is necessary.

- If there are large uric acid stones, it cannot be detected that throughout the course, some of the stones may dissolve and enter the bladder, which may further lead to blockage of the ureter.

- There is insufficient data regarding the use of Zyloprim during pregnancy. The use is not advised. The medication is found in breast milk, so it should not be taken during lactation.

- Children. Do not use the drug in children weighing <15 kg. 300 mg tablets should not be used in children weighing <45 kg.

- Before clarifying the individual reaction to allopurinol, you must be careful when driving vehicles or working with other mechanisms because of the possibility of dizziness or drowsiness.
